One thing I do see right off the bat is FreezeScreenSaver as this is known malware. Please go to Add/Remove and see if you can remove that. Also just uninstall Spyware Doctor if you did not purchase the program

I would like you to follow all the steps there, even though you have run some of these programs before I would like you to do it in the order given. Also do the online scans and run the other programs that PP recommends in Safe Mode. Have each program fix whatever it finds but make a note of everything found and where.
Then reboot in normal mode and run HJT again and post the new log here.
